Steridade (Universite Laval, Canada) and McCarley (Harvard Medical School) chose to author the work rather than prepare a compendium of contributed papers so that the material is presented with extraordinary coherence, with acknowledged emphasis on the work they know best (their own) and a conscious choice to omit areas of research that have been recently reviewed elsewhere.
